Why my contact form looks not good. See the attached screenshot. I checked the documentation and according to that it should look different. Could you please figure this out?

0which version of Invoice Manager is installed on your site?
if you use 1.2.1, it will look different. Here are the upgrade instructions from 1.1.0: http://www.joomlathat.com/documentation/invoice-manager/upgrading-from-1-1-0-to-1-2-0
